By signing the agreement, the University of Missouri-Columbia Office of Disability Services agrees to provide the following services:
- The Office of Disability Services will determine if you qualify as having a disability and are eligible for alternative formats of copyrighted materials.
- The Office of Disability Services, in cooperation with the Adaptive Computing Technology Center (ACT), will provide electronic text access and support for classes throughout the period you are enrolled at the University of Missouri-Columbia.
By signing the agreement, you agree to the following conditions:
- You must qualify to use alternative formats as determined by your disability service provider.
- You must own a physical copy of the text needed in alternative format.
- You must currently be registered as a student at the University of Missouri-Columbia and with the Office of Disability Services.
- Copyrighted electronic texts will only be available during the semester for which you are registered for that class.
- You will not revise, convert, disassemble, modify, sell, license, rent, loan or otherwise distribute the electronic text we provide to any other person.
- You agree that ACT may use your name, Paw Print ID, and student number for the sole purpose of setting up your web-based book account. A copy of this agreement will be given to ACT.
By their signatures, the University of Missouri-Columbia Office of Disability Services and the student agree to the above conditions. Please request a copy of this agreement for your records if you wish.
