Temporary Injury

If you have a temporary injury and need assistance with on-campus transportation, exams or note taking.

Please Read the Following:

  1. For assistance due to a temporary injury, such as a broken arm or leg, you may register with the Office of Disability Services by completing some brief paperwork and meeting with a staff member.
  2. You may be asked to provide medical documentation to verify your condition.  Acceptable documentation includes a doctor’s note, discharge summary or an emergency room visit report.
  3. You may also need to provide a copy of your schedule of classes at the time you register.  Please be sure to review it and bring it along with you to your appointment.
