Recency of Documentation

The provision of all reasonable accommodations and services is based upon assessment of the impact of the student's disabilities on his or her academic performance at a given time in a student's life. Therefore, it is in the student's best interest to provide recent and appropriate documentation relevant to the student's learning environment.

Documentation must address the student's current level of functioning and need for accommodations. All documentation must have been written within three years of the student's request for disability services. Because student's needs change throughout their academic history, it is appropriate to have an updated evaluation report.

However, the Office of Disability Services understands that the cost of diagnostic assessments is often expensive. Therefore, in the event that a student is only requesting the accommodation of extended time for exams, the following documentation criteria may apply:

· With the exception of the preceding recency criteria, the
  documentation must meet all other guidelines set forth by the Office
  of Disability Services.

· The evaluation must include assessments based on adult-norms.

· The student must show a history of receiving similar academic
  accommodations.
