Criteria for Documentation

The Office of Disability Services provides academic accommodations for students with diagnosed psychiatric disabilities at the University of Missouri - Columbia. The documentation that must be provided by the student must be typed, signed by a qualified professional and submitted on official letterhead. IEP or 504 Plans will bot be accepted as documentation of disability.

  1. The assessment must be within the last 12 months (1 year).
  2. The report must include the date of the diagnosis, a specific diagnosis using DSM-IV or equivalent criteria, and a review of pertinent history.
  3. The report must provide a summary of the assessment procedures used to make the diagnosis.
  4. The report must provide a description of the current symptoms, fluctuating symptoms and a prognosis.
  5. The report must provide medical information considered in a university environment including medication/therapeutic needs - outlining side effects of the prescribed medication, adjustment to medication, and a history of the student's compliance.
  6. The report must provide a statement of any substantial limitations of major life activities.
  7. The report should offer suggestions for academic accommodations and an explanation regarding each needed accommodation.
